To best serve all of our patients, we require 24 hours notice if you will be unable to keep a scheduled appointment. Failure to notify us will result in a $25 fee.
If you need a prescription refill, please contact your pharmacy. Please allow 72 hours for prescriptions to be filled. We cannot refill prescriptions on weekends, holidays or after office hours.
Lab and test results requiring immediate attention will be reported to you by phone. Other results will be reported via postal service or via MyChart. Our office will instruct you during your visit.
Billing and insurance
In order to better serve you, a copy of your insurance card is needed at each visit. All insurance co-payments and deductibles are due at the time of appointment check-in. We participate in many insurance plans. Patients will be responsible for any portion of charges not covered by insurance.
